Estradiol Cream in Thailand — what to actually ask for

You were prescribed estradiol cream back home. You walk into a Thai pharmacy, say "Estrace" or "Divigel" or "estradiol," and get a blank look. You leave convinced Thailand doesn't have it.

Thailand has it. You're asking for the American brand name, and the pharmacist is standing three feet from the same molecule under a different label. This page is the translation card.

First: which "cream" do you actually mean?

"Estradiol cream" in the US means one of three different things, and Thailand stocks them in three different ways. Figure out which one you're holding:

1. Vaginal estradiol cream → ask for Ovestin

This is what most people mean, and it's the easy one.

The US prescribes Estrace (estradiol) or Premarin (conjugated estrogens) vaginal cream. Thailand's shelf-standard vaginal estrogen cream is Ovestin (active ingredient estriol, 1 mg/g, with an applicator). Inference — estriol is a gentler estrogen than the estradiol in Estrace, but for vaginal dryness, atrophy, and painful sex it does the same job and is what Thai and regional gynecologists reach for first.

• Say to the pharmacist: "Ovestin" (โอเวสติน) — or show them the phrase card below.
• Legal status: over the counter. No prescription needed (details below).
• Inference — if a specific pharmacy is out, a hospital pharmacy or a

gynecology clinic will have it; it is not an exotic product here.

2. Estrogen face cream → this one's a clinic conversation

Tradition / context — In the US, the estriol or estradiol face cream for menopausal skin (thinning, dryness, loss of firmness) is made by compounding pharmacies to a doctor's prescription — it isn't a standard retail product anywhere, including Thailand.

What that means here:

• Thailand has no OTC registered estrogen face cream to point you at on a shelf.
• Inference — the same molecule is reachable two ways: (a) a **longevity or

aesthetic clinic** that compounds or prescribes it, or (b) off-label use of a registered estriol product. Both are real; both should go through a clinician.

Straight talk: don't improvise an estrogen face regimen off the internet. The dose that's cosmetic on skin can still be systemic. A clinic keeps it safe and legal.

3. Whole-body HRT → the whole shelf is open

If you're on systemic estrogen — menopause HRT or gender-affirming — Thailand is genuinely permissive, and this is a corner of the manifesto that's just true: therapies your home pharmacy rations, Thailand sells over the counter for pocket change.

Anti-counterfeit rule, from every expat who's done this: buy from big chain pharmacies or hospital pharmacies, not a random shop or a marketplace seller you can't verify.

The legal reality (you're right, it's OTC)

Estrogen in Thailand is classed as a "dangerous drug" — ยาอันตราย. That label sounds scary but it's just Thailand's behind-the-counter category:

• No prescription required. A licensed pharmacist simply has to dispense it and

ask about your symptoms — that's the whole gate.

• Bring your US prescription or recent bloodwork anyway. Inference — it isn't

legally required, but it turns a hesitant pharmacist into a helpful one instantly, especially for higher doses.

• It is not a controlled or special-control substance. This is ordinary

behind-the-counter medicine, like the country treats ibuprofen.

Before you self-manage — the straight safety box

• If you have a uterus and use systemic estrogen, you also need a progestogen —

estrogen alone raises endometrial-cancer risk. Vaginal estriol cream (Ovestin) is low-dose and local; systemic gel/pills/injections are the ones this matters for.

• Estrogen carries blood-clot, stroke, and hormone-sensitive-cancer risks that

are real and dose-dependent. This is why bloodwork and a clinician matter even where the pharmacy doesn't demand them.

• Don't guess doses off a forum. "It's OTC" means available, not "safe to wing."
• If you've been stable on a US regimen, you're mostly just re-sourcing the same

thing — the easy case. If you're starting fresh, start with a consult.
