Pharmacies in Chiang Mai: What You Can Buy, and Where, at 2 a.m.

The green cross is one of Thailand's great everyday mercies. A ร้านขายยา (ráan khǎai yaa — pharmacy) sits on nearly every commercial block, a licensed pharmacist stands behind the counter through the posted hours, and a striking amount of routine medicine is dispensed on the spot, at posted prices, in two minutes, without an appointment.

Here's how the system works, and where the counters are.

How Thai pharmacies work

Thai law sorts medicines into tiers, and the tier — not the shop — decides what you walk out with:

• General-sale medicines — paracetamol, antihistamines, rehydration salts, motion-sickness tablets, most topicals — are open-shelf. Convenience stores carry a legal mini-set of these for the 2 a.m. run.
• Pharmacist-dispensed medicines — a wide middle tier that includes many things Americans expect a prescription for — are handed over after a short consult with the on-duty pharmacist. Describe the problem; they'll ask sensible questions.
• Prescription-only and controlled medicines — psychiatric meds, sleep meds in the controlled classes, strong painkillers, and similar — require a doctor and usually a hospital pharmacy. A storefront that offers these without paperwork is telling you something about itself; walk on.

Two practical notes. Bring the generic name (the international one, not the US brand) for anything you take — the box here will carry the generic or a Thai brand. And pharmacist hours are roughly 9:00–21:00; outside them, the 24 h counters live inside the private hospitals on the hospital guide.

What it costs

Indicative posted prices, August 2026. A blister of paracetamol is ฿10–20. Common generics — antihistamines, simple antibiotics when dispensed, stomach medicines — mostly land ฿30–200 per course. Imported brands cost import-brand money. Nothing about the counter is a negotiation; prices are marked or quoted flat, and asking the price first is ordinary Thai manners.
